Communities we serve
Katrina Agnew serves as Chief Operating Officer for Attuned Care Comfort Hospice. She has a Bachelor’s degree in Business Administration, a Masters of Healthcare Administration, and a Fellowship in Public Health Leadership. Katrina has worked in the healthcare field for over 26 years, with 23 of those working in the hospice industry in several capacities. Her passion for hospice comes from the desire to ensure that she educates as many people as possible about its benefits. Empowering people to inform others about the beauty of hospice has been her mission throughout her career. Katrina is recently married to her husband Lerone. She has two Yorkies and a Miniature Pinscher, loves to travel, and enjoys spending time with family and friends.
Dianne Munevar, MPP, is a strategic leader with experience delivering data-driven analytics and strategy to organizations across the healthcare spectrum, with a keen interest in population health management. As the Vice President of Accountable Care, Dianne is responsible for helping to demonstrate the value of Attuned Care’s coordinated-care model to improve outcomes and deliver cost-effective care for all our members.
Prior to joining Attuned Care, Dianne was a Director at Avalere Health and a Health Services Researcher at RTI International where she worked primarily with The Centers for Medicare & Medicaid Services (CMS) on care delivery and payment reform for post acute care providers. She received her Masters Degree in Public Policy (MPP) from the Harvard Kennedy School, with a concentration in Social and Urban Policy and quantitative methods, and her B.A. in International Relations and Latin American Studies from Colgate University. Dianne lives in Evanston, IL with her husband and two kids, eats way too many PB&Js, and dreams of owning a 400-acre wildlife refuge.
Al Bromberg joined Attuned Care as CFO in 2015, supporting the company’s mission to improve healthcare for seniors by bringing his more than 35 years of experience as an accounting and finance executive. A CPA with an MBA from University of Illinois at Chicago, Al has successfully led multiple functions including accounting, financial reporting and analysis, human resources, operations management, compensation, and sales support. He and his wife Susan live in Deerfield, IL and are parents to three grown children. Al is also an avid White Sox fan and golfer and he serves on the Board of the United Way North-Northwest and on the Deerfield Planning Commission.
Benjamin Berk is dedicated to building teams who provide the kind of care that would have made his grandparents smile. Benjamin is an experienced medical doctor, information technologist, and business leader, and is honored to be CEO of Attuned Care.
He is an “interdisciplinarian,” bringing together the clinicians, caregivers, and specialized experts who collaborate to deliver outcomes that matter. Previously, Benjamin was VP Population Health at Iora Health, a primary care startup. He was also Principal at Boston Consulting Group (BCG) and has been an adviser to Oliver Wyman, working with health systems and startups in the US and UK to build accountable, population health-based models of care. Dr. Berk is an Adjunct Instructor at The Dartmouth Institute for Health Policy and Clinical Practice.
He received his MD and Masters in Management Science and Engineering from Stanford and his BA in Information Age Studies from Dartmouth College. Benjamin lives with his wife and their three children, and escapes to places out-of-doors.
Franna Carlson, RN, is a licensed nursing home administrator. She has been a registered nurse for 34 years and has experience in medical, surgical, acute and hospital care. Franna is well versed in skilled nursing management and has been in assisted and supportive living management for the past 14 years. She always wanted to be a nurse. Even as a child when asked to draw a picture of what she wanted to be when she grew up, she drew a nurse. Franna loves to help others, especially those who have limited ability to help themselves.
She is also mom to three grown kids, grandma to seven grandkids (six girls and a boy), and has two mini poodle mixes that she and her husband Mark rescued about six years ago. Mark and Franna enjoy summers at Woodhaven Lakes, travelling (especially to Florida), and going on cruises. Franna loves country music and reading in her spare time, but what she loves most is spending time with her kids and grandkids.
Bruce Doblin is a primary care physician with experience starting up and running both for-profit and not-for-profit healthcare programs. Prior to joining Attuned Care, Bruce practiced community-based primary are for more than 25 years in his own practice. He was the founding director of three hospice and palliative care programs, served as healthcare consultant for the State of Illinois, directed a not-for-profit healthcare advocacy organization and worked on healthcare policy in both Washington, D.C. and Illinois. Bruce was born and raised in Chicago and has been practicing Vinyasa yoga for the last 20 years. He recommends the discipline to his patients because it is the perfect balance of strength, balance and flexibility training and offers meditative benefits as well.
Madlene Moore-Condon MSN, BSN, RN is an executive nursing leader with a profound knowledge of the healthcare industry. She works in the post-acute setting with the senior citizen population to address healthcare needs before they are impacted with life-changing decisions. Madlene is adept at partnering with clients to create value alignments that positively influence their healthcare patterns, clinical outcomes, and costs. She utilizes her expertise in process improvement and organizational change to analyze and identify inefficiencies and then deliver strategies for targeted improvement. Madlene is an innovative, results-oriented professional who is energetic, adaptable, and skilled at addressing challenges with a deep dedication to Attuned Care and its people.
Madlene joined the post-acute setting six years ago. With parents who are aging in place, her primary goals are to maintain wellness and provide autonomy for them as long as possible. She has been married to her husband Kevin for 29 years and has two grown children. Madlene loves sports, plays tennis, and has a mean forehand. In her free time, she also enjoys traveling and spending time with family.
Martin is a healthcare leader with over 30 years of experience. Throughout his undergraduate studies, Martin worked as a Nursing Assistant in a skilled nursing facility, which inspired him to dedicate his career to helping improve the quality of life for this precious and growing segment of the population. While in graduate school, he worked as a Case Manager coordinating in-home services for seniors living in a four-county area in Central Illinois. Upon completion of his graduate studies at the University of Illinois at Urbana-Champaign, he became a Licensed Nursing Home Administrator. He spent the next 15 years of his career in that capacity, in both publicly traded and not-for-profit settings. He left the skilled nursing environment to serve as Executive Director and later Regional Director of Operations for a supportive-living organization. He joined the Attuned Care team in November of 2014 where his broad background in senior-living operations has helped him build constructive relationships with the senior communities Attuned Care serves.
Martin and his wife Sharon enjoy travel and socializing with family, friends, and healthcare colleagues. Martin’s interests include sailing, singing, camping, cooking, fitness, and Boxers. He completed 15 Ironman triathlons and 13 marathons during a 13-year span. The Ironman slogan is “anything is possible,” and Martin adds, “finish what you start.” He feels that one of the greatest gifts that comes with working with senior citizens is the opportunity to get a glimpse into the amazing lives of the people we serve, as well as of those we serve with.
Paula DeFrees brings 20 years of experience as a physical therapist to her role as Director of Therapy for Attuned Care, and she has spent the last 20 years in home care. Paula earned two Bachelor of Science degrees: one in Zoology from Eastern Illinois University and one in Physical Therapy from Chicago Medical School. She is currently enrolled in a program to earn her Doctor of Physical Therapy degree. Paula was drawn to physical therapy by the close relationships naturally formed by the nature of this work and the direct impact she could make on another’s life.
Paula’s love for animals has her planning to start a Therapeutic Riding Program once she reaches retirement. She and her partner Deb have a 12-year-old dog named Sydney and have enjoyed world travels, visiting multiple countries on five of the seven continents. Paula frequently visits with her sister and parents in Streator, IL and she spends her spare time with Deb at their second home in Palm Springs, CA.
Brian Cloch is a healthcare visionary and leader who has dedicated more than 30 years to improving quality of life for older Americans. He has owned and operated a variety of senior housing communities, transitional care and skilled nursing facilities, as well as supportive and independent living senior housing. He is also a partner in ancillary care businesses including home health, rehabilitation, hospice, and medication management. Brian is the Founder and former President of the Affordable Assisted Living Coalition and the former Chairman of the Illinois Department of Professional Regulation Nursing Home Administrator licensing board. In his free time, he loves to spend time traveling with his family and has successfully completed an ironman triathlon. Brian has three adult children and lives in the north suburbs of Chicago with his wife.
